If the nurse simply asks the client his or her name, the reply may be yes, because the client did not hear, did not speak English, or simply wanted to be misleading. Another nurse and client family members may confirm identity as a last resort, such as if a client is in a coma, but checking the ID and asking for name and birth date are the recommended guidelines for alert clients.
