Answer to Question 1

A
Feedback:
A hemolytic disease of the newborn, erythroblastosis fetalis, can occur when an Rh-negative mother is pregnant with an Rh-positive fetus, resulting in Rh sensitization (Rh incompatibility). The disease does not occur in the first pregnancy, but occurs with increasing severity in future pregnancies. The condition causes the woman's antibodies to destroy fetal red blood cells. Phototherapy is used to treat milder cases after birth. If such measures do not help, a series of exchange transfusions probably will be performed. In severe cases, the fetus may receive an intrauterine transfusion before birth.

Answer to Question 2

B
Feedback:
Nurses should turn clients frequently to prevent skin breakdown. Prolonged bed rest is dangerous for clients with musculoskeletal disorders because of the increased risk for complications such as skin breakdown, contractures, constipation, and thromboembolism (also referred to as deep vein thrombosis). When bathing clients, nurses should minimize the use of soap. A high-protein diet should be served to the client.
