Answer to Question 1

A, B, D, E
Feedback:
SBAR stands for Situation: who are you and why is the contact being made? Background: pertinent information, as listed above. Assessment: what is the situation as reported by nursing staff? Recommendations: what should be done for the client? Why is the transfer being made or what instructions are you seeking from the healthcare provider? These recommendations must be clearly communicated to all staff involved.

Answer to Question 2

A
Feedback:
If the client is unable to cough up sputum, the physician may order that either the nurse or the respiratory therapist obtain a specimen by bronchoalveolar lavage. In this procedure, sterile saline is instilled into a bronchus. Then, cells and fluid from the bronchioles and alveoli are removed by endoscopy along with the saline. The cells are analyzed in the laboratory, most often to diagnose pulmonary tuberculosis (TB).
