Answer to Question 1

B
Feedback:
A sensor is placed on a finger, toe, or earlobe, not the hand.

Answer to Question 2

D
Feedback:
The nurse should assess the perineal area for discoloration and swelling when a client develops postpartum hematoma. The nurse may give analgesics to relieve discomfort. The nurse may give the client cold, not warm, compresses or a medicated pad. Sometimes, compresses soaked in a magnesium sulfate solution can be used. A birth attendant, not the nurse, performs an incision and ligation of a blood vessel, if necessary.
