Answer to Question 1

A
Feedback:
The nurse should instruct the client to avoid excess intake of fluid because the medication is flushed out before taking effect. The medication may leave the body before it has a therapeutic effect. Toxic level of medication may accumulate in the body if the kidneys are unable to filter out excess medication. Chemical changes also may form substances that are harmful to the body if it is unable to dispose of them rapidly enough.

Answer to Question 2

B
