Answer to Question 1

D
Feedback:
Removing the self-adhesive sheath and exposing the penile skin to the air at intervals helps avoid prolonged pressure that may cause skin breakdown. The catheter must be removed from the bag connection and swabbed with antiseptics several times during the day and not only once. The drainage port should be opened to allow the urine to drain out of the bag several times in a day, not just at the end of a 24-hour period. The leg bag should not be washed with hot, soapy water but rather with warm water. After rinsing it should be hung up to dry, not spread flat.

Answer to Question 2

B
Feedback:
The inside of the gown or apron is clean. The outside of the gown or apron is contaminated. If the nurse is wearing long sleeves, the sleeves are rolled up above the elbows before putting on the gown or apron. A supply of clean gowns or aprons is kept just outside the door of the client's room.
