Assessing vital signs every 4 hours is important, but is not the single most effective nursing measure to prevent the spread of disease. Wearing a mask is necessary only with airborne microorganisms when a client is on isolation. Raising the temperature in a client's room would encourage the growth of microorganisms.

The nurse would require a physician's order to provide a heat application to the client. Changing the client's position, providing a back rub, or massaging the client's hands are comfort measures that the nurse could provide to the client without a physician's order.
