Answer to Question 1

D
Feedback:
Clients have the right to know the name, action, and possible side effects of medications administered to them. They also have the right to refuse to take medications, unless a court order gives healthcare workers the right to administer medications without the client's consent. (If clients are endangering themselves or others, medications may be given against their will.) Clients also have the right to request the generic form of prescribed medications, if available. Generic forms of medications are often less expensive than their brand name counterparts.

Answer to Question 2

D
Feedback:
The nurse should document the rapid eyeball movement from side to side observed in the client as horizontal nystagmus. Delirium tremens is the condition wherein the client has vivid and terrifying auditory, visual, and tactile hallucinations; it is an indicator of a severe toxic state during stage 3 of unmanaged alcohol withdrawal. Spider angioma and palmar erythema are signs of chronic alcoholism. Spider angioma is a condition in which the client has spidery veins on the nose and cheeks. Palmar erythema is the condition of thickened and reddened palms.
