Answer to Question 1

D
Feedback:
The client most at risk is the 88-year-old client on steroid medications. A very old age or a very young age increases vulnerability. A client in the emergency department with chest pain has just arrived in the facility and has not had enough time to be considered to have a nosocomial infection. The pregnant woman is at low risk. The 66-year-old client with chronic pulmonary disease is second in line for an infection because of a chronic respiratory condition, but is not as much as the 88-year-old client.

Answer to Question 2

A
Feedback:
Physical causes include the mechanical stress of trauma, surgical incision, or tumor growth. Fatigue, fear of the unknown, and lack of knowledge about pain management can cause further muscle tightening.
