Answer to Question 1

B
Feedback:
Active intervention must occur. If no one intervenes in the process of CD, it usually continues. Someone must recognize the condition. The first person to do so is very often someone other than the chemically dependent person. Treatment of CD often responds to structured therapy. Many therapies assist chemically dependent people to lead a successful and productive life. Many such programs use the 12-step approach of Alcoholics Anonymous (AA). They may also apply principles of behavior modification.

Answer to Question 2

C
Feedback:
Mechanical debridement utilizes wet-to-moist or wet-to dry dressings. As the dressing dries and is then removed, some of the necrotic tissue that adheres to the dressing is removed with the dressing change. Autolysis is a method that uses the body's own enzymes and moisture to rehydrate the wound and dissolve wound slough. Enzymatic debridement involves the use of commercially prepared chemical enzymes applied to the necrotic tissue of the wound. Surgical debridement is normally done by the physician, and either a scalpel or scissors is used to remove slough and eschar.

Answer to Question 3

C
Feedback:
The nurse should check the client's core temperature frequently to ensure that the client's core temperature does not fall too low. The treatment should be stopped before the temperature reaches the prescribed level because the temperature continues to drop even after the treatment is discontinued. Blood pressure need not be monitored continuously. The nurse should arrange for a bath blanket to be ready to use in case the client experiences chilling.
