Answer to Question 1

B
Feedback:
Vitamin K is given to prevent bleeding problems. Eye prophylaxis is used for infants born to mothers with gonorrhea or chlamydia affecting her reproductive organs. Also, the first vaccination against hepatitis B is given, and Standard Precautions are used when handling the baby or caring for the mother.

Answer to Question 2

C
Feedback:
When preparing a client for skin testing, the nurse should have an ice pack readily available to treat the symptoms of anaphylaxis, if any. Applying an ice pack to the site helps reduce the rate of absorption of the antigen into the system, thus reducing the risk for anaphylaxis. The nurse should keep epinephrine in 1:1,000 concentrations, not 1:10,000 concentration, to counteract the vasodilation that occurs during anaphylaxis. The client should be encouraged to wear a facemask in cold weather and not during skin testing. Intradermal injections cause little pain, if any, not severe pain, because the needles used in these tests barely penetrate the skin's surface.
