Answer to Question 1

D, E, F
Feedback:
The client with pruritus should rinse clothes in clear water and avoid using fabric softeners, starch, and antistatic chemicals. The client should lightly slap instead of scratch the areas, and keep the fingernails short to prevent scratching the rash. Baths and showers should be cool, nonallergenic makeup can be worn, and cotton instead of wool clothing should be worn.

Answer to Question 2

D
Feedback:
A bed board may be placed under the mattress to support the body in correct alignment. A bed cradle is a frame used to prevent the bedclothes from touching all or part of the client's body. Side rails (safety rails) are bed rails that not only prevent clients from falling out of bed, but also help the client to change position while in bed.

Though Candida and Aspergillus species are the most common fungal pathogens causing invasive fungal disease in the immunocompromised, infections due to previously uncommon hyaline and dematiaceous filamentous fungi are occurring more often today. Rare fungal infections, once accurately diagnosed, may require surgical debridement, immunotherapy, and newer antifungals used singly or in combination with older antifungals, on a case-by-case basis.
