Answer to Question 1

B
Feedback:
The nurse should suggest that the use of condoms is the best way to prevent STIs. Sex education helps adolescents to know about sex, sexuality, and changes occurring in their bodies, whereas birth control methods help to prevent pregnancy; this knowledge will not help the client minimize the risk of STIs. The nurse should address the client's fears instead of asking him to refrain from sexual activity.

Answer to Question 2

C
Feedback:
A woman's vagina is acidic because of its normal flora. The alkaline environment of seminal fluid helps neutralize the acidic vaginal pH and maintains sperm motility and viability. The acrosome of the sperm contains enzymes that can dissolve the tough cell wall of the ovum. The ejaculatory ducts empty into the urethra and receive secretions from the prostate gland.

It is tightly attached to the skin below it, so it compensates for the increased area by wrinkling. This happens to the hands and feet because they have the thickest layer of dead keratin cells.
