Answer to Question 1

B
In Option b, the entry contains the problem, the assessment, subjective comments, observations, and the plan. Option a does not define the patient's pain. Options c and d reflect opinions of the nurse.

Answer to Question 2

D
Option d is validating the patient's perception of pain. Option a minimizes the patient's feelings, and Option b may cause the patient to feel rejected by the nurse. Making a stereotypical com-ment (Option c) is never therapeutic.
