Collaborative prob-lems do not fall within the definition of nursing diagnoses. Option b is not true, and Option d is true of the nursing diagnosis phase of the nursing process.

Safety is the first priority for this patient who is cognitively and visually impaired, wanders, and is unsteady. Laboratory results (Option a) should be monitored, but safety is the priority. Options b and d are of concern but are not immediate priorities.
