A client in a coma or a client requiring neurological monitoring of cerebral perfusion pressure and intracranial pressure is beyond the educational preparation of a practical nurse. The practical nurse is taught to assess a client's neurologic status but does not have the training to perform a detailed neurologic assessment. A practical nurse graduate may teach skills to a client, such as postural drainage. The registered nurse should write a detailed teaching plan and teach the client on how to care for a neurological shunt.
