Answer to Question 1

A, B, D
A successful telehealth program will result in improved patient satisfaction, fewer admissions for chronic conditions, and satisfaction with the technology as reported by remote providers.
Police reporting that they encounter fewer mentally ill patients in crisis may or may not be related to having access to mental health providers remotely. That is one possibility for fewer crisis situations.
Third-party reimbursement would be ideal but is not yet the reality.

Answer to Question 2

D
An active failure is an unsafe act committed by a person in direct contact with the patient. This is directly opposite a latent failure, which is caused by a systems problem.
An active failure is caused by a person in direct contact with the patient, not by a systems process.
An active failure is caused by a person in direct contact with the patient, not by a workplace practice per se.
An active failure is an unsafe act committed by a person in direct contact with the patient.
