image reveals that OML was not quite perpendicular to the IR

Rationale: The orbits appear only three quarters full with the petrous pyramids indicating the chin was not tucked enough; OML was not quite perpendicular to the IR.

Answer to Question 2

Rationale: An ambulatory chest should be evaluated for inclusion of the entire lung field, no rotation at the SC joints, removal of scapulae from the lung field and a minimum of 10 ribs above the diaphragm.
