Patients diagnosed by Pap smear as having severe cervical dysplasia or intraepithelial carcinoma of the cervix require conization to remove the lesion and rule out invasive carcinoma. The biopsy, obtained with a laser, scalpel, or cervitome (cold knife conization), includes the squamocolumnar junctions of the ectocervix and is tapered to include the endocervical canal to the level of the internal os.
