Feedback: The use of restraints is warranted only when the client's safety is in jeopardy and other, less restrictive measures have not been effective. The nurse does not base her decision on the client's mood or court order. A physician's order for use of restraints does not mean that they are appropriate in every situation; this is based on nursing judgment.
