To provide useful clues about causes of disorders, subgroups with high and low rates of mental illness were included. Residents in institutions (including nursing homes, psychiatric hospitals, and prisons) were included. The rarer disorders were thought likely to occur more commonly among institutionalized individuals.

The first step in treatment of a client who is anorexic is nutritional rehabilitation. Adequate nutrition to stop starvation must be the top priority. In some cases, force-feeding may be necessary. Once adequate nutrition has been established, psychotherapy, maintenance, and finally, follow-up care can proceed.
