Answer to Question 1

B
The most appropriate outcome for the nurse-client working phase would be that the client will develop insight and understanding of how current maladaptive methods of interacting and coping contribute to the client's problem. The client identifying and agreeing to work on a priority issue during treatment, and demonstrating congruent verbal and nonverbal behaviors indicating establishment of trust and feeling of nonjudgmental acceptance are outcomes that occur during the orientation phase of the nurse-client relationship. The client consistently demonstrating congruent verbal and nonverbal behaviors indicating readiness to independently resume preepisodic social role functions is an outcome that would occur during the termination phase of the relationship.

Answer to Question 2

A
Suicide is a major physical risk for individuals diagnosed with BPD. Although attempts are much more common than completed suicide, there is a real risk of suicide success, either intentional or unintentional. Because of extreme emotional lability and frequent association with substance abuse, suicide may be very difficult to predict or prevent in BPD.
