These plans provide a means to record assessment data, provide a list of nursing diagnoses drawn from the data, record anticipated outcomes of nursing interventions, and present a plan of care that is a statement of nursing interventions to be used. Finally, the nursing care plan provides documentation of the observed outcome, which can be compared to the anticipated outcome. This last item is generally placed in the final column known as the evaluation.
