The nurse would assess for risk of suicide. The nurse's assessment of the client's slow speech, crying, and statement of feeling depressed indicates to the nurse that the client might in fact be depressed. Given the history of bipolar disorder, compounded by a recent major surgery, the nurse would recognize that the client is at risk for suicide.
