An 8-year-old child was recently prescribed a stimulant to help manage the symptoms of attention deficit/hyperactivity disorder (ADHD).
 
  The school nurse, who visits the school weekly, provided the parents and teacher with a behavior log and instructed them to record the child's behavior in it for the nurse's review and for sharing with the psychiatrist. The rationale for this is to: A) Make sure the child takes the medication.
  B) Keep the child from being too active.
  C) Include the teacher and family in the plan of care.
