A female patient is in the clinic after being raped by a stranger. She seems to be having problems staying in the present and answering questions. The following short-term goal has been established by you and the RN and the patient/client:
 
  A) Resolve the feeling of fear and anxiety related to the rape
  B) Insist client/patient clean her own wounds to begin the acceptance process of the rape
  C) Have patient/client verbalize her feeling about the rape
  D) Have nurse prepare the care plan for the patient/client to decrease her need to make decisions right now
