Answer to Question 1

ANS: D
Safety needs of the client and other clients are a priority. The client is less likely to hurt himself or
others when several staff take responsibility for providing limits. The explanation given the client
should be simple and neutral. Simply being told that others can provide the control he is losing may
be sufficient to help the client regain control. Option A: Running after the client will increase his
anxiety. Option B: More than one staff member will be needed to provide physical limits if they
become necessary. Option C would be futile; a client in panic processes information poorly.

Answer to Question 2

A, B, C, D, E, F
Rationale: The nurse will be filling roles of case manager as well as family therapist; thus all the
tasks listed in the options would be appropriate for the nurse to undertake.
