smile while repeating the client's name until the client focuses on the nurse. Once the nurse has the
client's attention, gently touching the client and asking what she needs are appropriate. Striking out
usually signals fear or that the client perceives the environment to be out of control. Option D would
generate greater fear.

This is a highly lethal method with little opportunity for rescue. The other options are lower lethality
