Answer to Question 1

B
The nurse needs to make sure that any herbs the patient may take are not harmful when taken in conjunction with the prescribed antidepressants. Patients adhere to their own belief systems, so admonishing the patient not to take herbals will not be effective.

Answer to Question 2

2
Child abuse and neglect are hitting crisis levels.
