Rationale: CISD was originally formulated in the 1980s in response to an air disaster in Washington, DC. The original approach, known as critical incident stress debriefing, was intended to include four major elementson-scene crisis intervention; post-incident small group discussion (known as defusing); a more formalized several-phase group discussion, which included debriefing; and follow-up support services. In practice, however, only one element, the group discussion, often to the exclusion of the others, became the focus of disaster mental health intervention. Its ease of use and the perception that it prevented PTSD led to its popularity. Although the small group meeting has both psychologic and psychoeducation elements, it should not by itself be considered psychotherapy.
