Answer to Question 1

4
Rationale: Clinicians use Axis III to record physical disorders and medical conditions that must be taken into account in planning treatment at the time of admission. Axis IV is used to identify problems or issues of a psychosocial and environmental nature.

Answer to Question 2

2
Rationale: Schizophrenia has an 80 chance of heritability. At present there is no specific genetic test available. Clients and families should be presented with information about the benefits of genetic research for future generations of people with schizophrenia. Telling the client there is nothing to worry about is false reassurance and is not therapeutic. Acknowledging the client's distress is appropriate, but the nurse must address the client's concerns directly.
