Members of a nursing student organization volunteer to serve lunch at a homeless shelter used by many individuals who are severely and persistently mentally ill.

Because many have dual diagnoses, the students should expect to comply with shelter policies and set limits by refusing services to individuals who are:

  2. Intoxicated
  3. Disheveled
  4. Disaffiliated

Answer to Question 1

ANS: 2
Homeless shelters rarely have the staff to serve individuals who are actively hallucinating or de-lusional, who are intoxicated, or who display overtly threatening behavior. Because many se-verely and persistently mentally ill are homeless and because many self-medicate with alcohol, the students will likely see a relatively high number of intoxicated clients who will be refused services. Option 1: Developmental disability is not a barrier to obtaining services at a shelter. Op-tions 3 and 4 are not barriers to obtaining services at a shelter; in fact, most shelter clients mani-fest these characteristics.

Answer to Question 2

ANS: 1
The expected outcome that he will not verbalize intentions to harm others is directly related to the problematic behavior. Option 2 is incorrect because the client was demonstrating psychotic behaviors at the time he threatened neighbors. Options 3 and 4 are important but not directly re-lated to the behavior-prompting admission.
