Answer to Question 1

ANS: 2
Because the abuse has been long-term and is increasing in intensity, the client needs to state her awareness of being in danger. Once the client accepts this fact, she may be increasingly ready to make further plans to extricate herself. 1. The victim is not at fault for abuse. 3. This is not prac-tical, since she has not begun to pursue litigation. 4. Retaliation is not an effective means of re-solving the problem.

Answer to Question 2

ANS: 4
Self-examination is required in order for the nurse to be objective and therapeutic in providing care. 1. This is not the initial step. 2. Removal is not always recommended. 3. This may not be required, depending on the situation.
