Similarly, if the client uses denial or rationalization as he or she discusses the dis-order, the nurse can assess the presence of cognitive disorder. The other options, while valid questions, do not assess for cognitive distortion.

Having enlarged breasts at the time of puberty and thereafter is a characteristic finding in indi-viduals with Klinefelter's syndrome. 1. Aspermatogenesis is an expected finding. 2. FSH level is expected to be low. 3. This is not an expected finding.
