Answer to Question 1

ANS: 3
Finding no pleasure in living should suggest the need for further assessment of suicide potential. Safety needs take priority over problems suggested by other data collected.

Answer to Question 2

ANS: 1
When the presenting symptoms are absent, the nurse can evaluate the problems as resolved. 2. Most clients with adjustment disorders do not require medication, so this is not a good indicator. 3. Data do not substantiate this as having been a problem. 4. This could indicate the client is overeating as a means of dealing with stress.
