A client has been diagnosed as having acute pain disorder associated with psychological factors.

She became suddenly incapacitated by pain 3 months ago and has since been unable to provide her three children with an optimal level of parental support. The children report that Dad tries, but things aren't as good as they were before Mom got sick. These data suggest the need to complete the nursing diagnosis:
  1. Risk for loneliness
  2. Decisional conflict
  3. Interrupted family processes
  4. Risk for delayed development

Question 2

The nurse has identified a diagnosis of dissociative identity disturbance for a client who has stated that he is unable to distinguish between himself and his surroundings. An outcome related to this diagnosis is that the client will:

  1. Suppress feelings of dissatisfaction
  2. Refer to himself as the client
  3. Use aggressive behaviors to meet needs
  4. Identify increasing anxiety

Answer to Question 1

ANS: 3
Defining characteristics include changes in mutual support, availability of affective responsive-ness and emotional support, satisfaction with the family, somatic complaints, etc. The etiologic factor could be established as a shift in the health status of a family member. Data are not present to fully support the other options.

Answer to Question 2

ANS: 4
Dissociative identity disturbance is exacerbated when the client's anxiety escalates. Identifica-tion of increasing anxiety permits the client to exercise anxiety-management strategies and pre-vent dissociation. 1. The client should be expressing such feelings to others. 2. The client should refer to himself in the first person. 3. Use of assertive behavior is desirable.
