Answer to Question 1

Ans: C
Delusional disorders can be precipitated by psychosocial factors such as severe stress. They are not a consequence of electrolyte imbalances, and genetic links are unclear. Neurological etiologies are usually due to disease of the limbic system and basal ganglia rather than traumatic brain injury.

Answer to Question 2

Ans: A
The client is expressing fear and anxiety resulting from a perceived threat. He is confused about his role and/or identity, but there is no evidence of a disturbance in sensory perception (ie, hallucinations). Defensive coping is not in evidence, and impaired social interactions are secondary to the client's immediate fear.
