Answer to Question 1

A
To effectively use a seamless continuum of care, a nurse must have case management skills with which he or she can coordinate care using available and appropriate community resources. Psychosocial assessment and physical assessment are functions that can be fulfilled by another health care worker. Patients' rights advocacy is one aspect of case management.

Answer to Question 2

D
The reticular activating system (RAS) serves as a screen that allows individuals to tune out some stimuli and attend to others. When RAS disruption occurs and the individual is unable to tune out stimuli, the individual is unable to sleep. Lack of sleep can produce psychotic symptoms. The other structures are not involved in perception of sensory stimuli.
