Answer to Question 1

Answer: 3
Explanation: The paradigm shift for nursing care occurs when nurses develop a partnership with patients and families, emphasis on individualized care and the fostering of hope for recovery. Neoprene restraints, rounding every 2 hours, and locked glass windows have more to do with the old paradigm of consistency, control, and vigilance.

Answer to Question 2

Answer: 2
Explanation: Providing positive feedback and offering to share the results with other units would validate the team's efforts and foster empowerment. Taking the project to the hospital committees may be necessary but would not likely foster empowerment. Revising the project would undermine the staff's efforts and analysis. If it needed to be revised, the manager might make some helpful suggestions rather than assume responsibility for it. Stating that the physicians might not agree with the project might be true, but the nurse manager should advocate for her staff if she believes in their project.
