Answer: Who or what performed the action is unknown; readers do not care who or what performed the action; a long process is performed by only one person; identifying the performer of the action would be unnecessarily embarrassing.

Answer to Question 2

Answer: a sentence that lacks articles, prepositions, and linking verbs and that sounds like telegram phrases
