A patient's open wound is healing by third intention. The nurse charts:
 
  1. Wound intact and draining a moderate amount of serosanguineous fluid.
  2. Wound approximately 2?3?  3?3?, granulation tissue visible, draining serous fluid.
  3. Wound 4?3? in diameter, open with jagged edges, draining sanguineous fluid.
  4. Wound 6?3? long with well-approximated edges, reddened around incision, no drainage.
