Answer to Question 1

4

Rationale: The nurse should not isolate herself or himself with a potentially violent client. The door to the client's room should remain open when giving care. The client should be placed in a room near the nurses' station and not at the distant end of a corridor. The nurse should never turn away from the client. A security officer or male aide should be readily available if there is a pos-sibility of imminent violence.

Answer to Question 2

3

Rationale: In the child with intussusception, bright red blood and mucus are passed through the rectum, resulting in what is commonly described as currant jelly stools. The child classically presents with severe abdominal pain that is crampy and intermittent, causing the child to draw the knees in to the chest. Vomiting may be present, but not projectile. Scleral jaundice and pale-colored and hard stools are not manifestations of this disorder.
