The initial assessment of a patient who was admitted with a gastrointestinal inflammatory disease revealed that the patient had hyperactive bowel sounds.
 
  The follow-up assessment finds that the bowel sounds are still hyperactive but are now audible without a stethoscope. The nurse will correctly chart:
  1. Low-pitched bowel sounds  D.
