Answer to Question 1

3. I often put the children in the tub to play..

Rationale:
Toddlers are at risk for drowning, even in small amounts of water. The nurse would want to teach the parent who uses the tub as a place for toddlers to play unsupervised. Taking the children out of the tub or talking in the bathroom demonstrates an awareness of the need for supervision for toddlers. The parent who does not leave toddlers along in the bathroom is also demonstrating an awareness of risk.

Answer to Question 2

2. I expected this to happen, but not right now..

Rationale:
The client is not being realistic by stating that this was not expected now. The nurse realizes that he may continue to drive or may become depressed. A family conference could help assist the client to make safe plans for going out at night. Limiting driving to daytime hours, planning for help to go out at night, and anticipating more hours of daytime in the summer demonstrate acceptance and compliance by the client.
