2. Have the client sleep on 2 or 3 pillows a night

Rationale:
The client should sleep with the head elevated if a cough becomes troublesome when in supine position. A cough induced by an angiotensin-converting enzyme inhibitor- will not be relieved by cough medication. Sitting up at an 80angle would be effective but would be too uncomfortable for the client. A sedative-hypnotic medication would put the client to sleep, but it does nothing to address the client's cough.

Rationale:
The nurse is always honest in replying to a client. The nurse tells the client that the tests will provide the actual results but that he is in a low risk category. Telling the client that he does not have the disease is not wise as, Japanese or not, he could have BPH. Asking a client where he got that idea is demeaning.
