A family with 4 children has just learned that their third child, age 5, has been diagnosed with ADHD. The nurse has assessed the child and the family and is planning care with them.

Which of the following statements by the mother leads the nurse to a diagnosis of compromised family coping? 1. We need to alert the teachers at school so they will know how to give Tommy's medication..
   2. I'm not sure if we should let Tommy act in the school play..
   3. Will he have to be put in an institution?
   4. I don't know how to tell the rest of the family or how we will manage the other children..

Question 2

The nurse is working on a unit that is shorthanded due to another nurse's illness. The manager was unable to replace the nurse, so the extra clients were assigned to the remaining nurses.

The manager was able to get the help of unlicensed assistive personnel (UAP) from the house pool to help on the unit. In order for effective care to be given to the clients, the nurses would: 1. Delegate tasks to the UAP.
   2. Explain to the manager that care may be compromised if another nurse does not work the shift.
   3. Tell the clients their care will be sparse.
   4. Tell the manager what tasks the UAP should undertake.

Answer to Question 1

4. I don't know how to tell the rest of the family or how we will manage the other children..

Rationale:
The mother does not have a positive outlook on this situation and appears indecisive and ashamed. This family will need assistance with coping with Tommy and continuing on with life. Alerting the teachers at school is a positive action and a way to protect the child. The school play will be a decision made with the teachers, but the mother's statement does not suggest poor coping. Asking if Tommy will need to spend time in an institution is reasonable at this time.

Answer to Question 2

1. Delegate tasks to the UAP.

Rationale:
The nurses would delegate to the UAP tasks such as taking and recording vital signs, performing a.m. care, and ambulating clients to ensure that all clients receive appropriate care. The goal for the unit is to meet the needs of the clients; complaining to the manager will not accomplish this. UAPs can be very helpful because they are trained to complete tasks that the nurses would not have time to complete, thus accomplishing the goal of care for all clients on the unit. To alleviate clients' anxiety, the nurse may tell them that extra help is coming for the shift. Nurses delegate tasks directly to the UAPs rather than telling the manager what needs to be done.
