The nurse is conducting a physical assessment of a 49-year-old client during an annual assessment. The nurse plans to conduct a nutritional and exercise assessment because:

2. Middle-aged clients have slower metabolisms.

Rationale: The middle-aged client's metabolism begins to slow, and the client may need nutrition and exercise guidance to maintain optimum weight. While some chronic diseases are controlled by nutrition and exercise, t he specific need of this age group is knowledge regarding changes that are occurring. Protein intake for most normal clients remains constant throughout the life span. Healthy middle-aged adults do not necessarily get sick more often than other age groups.

1. Acute Confusion

Rationale:
The definition of acute confusion is the abrupt onset of a cluster of global, transient changes and disturbances in attention, cognition, psychomotor activity, level of consciousness, and/or sleep/wake cycle.. The client may have anxiety as a result of confusion. Autonomic dysreflexia is a syndrome of clients with spinal cord damage. Ineffective coping is also a result of the confusion.
