Answer to Question 1

4. The client will regain mobility.

Rationale:
The client has sustained multiple lower extremity fractures. A goal of care for this client is for the client to regain mobility. An adequate urine output is important, but healing the client's fractures needs to be a goal of care. Participating in self-care activities and being discharged to home are also important however, since the client sustained multiple fractures to the lower extremities, one goal of care must be focused on the client regaining mobility.

Answer to Question 2

B
Tolerance is when a specific dose of medication loses its effectiveness and the patient requires larger doses to produce the same level of analgesics. Physical dependence occurs when the body physiologically adapts to opioids. When the opioids are discontinued, withdrawal symptoms occur. Addiction is behavior of overwhelming involvement with obtaining and using a drug for other than approved medical reasons. Pseudoaddiction is not a term used with opioid use.
