Answer to Question 1

D
Interventions to promote airway clearance include effective coughing techniques, the use of postural drainage and chest physiotherapy, and ensuring adequate hydration.

Answer to Question 2

2. Discuss the possibility of an unconscious distancing response with the coworker.

Rationale:
Distancing is a normal self-protective mechanism to avoid the pain of losing someone the nurse cares for or caring for a client with appliances. Because it is often a subconscious action, helping the nurse to recognize what is happening is often all that is needed to correct the situation. It also provides the nurse with someone to talk to about feelings. Offering to trade assignments will not help the employee face the problem. Reporting to the supervisor is necessary when the client is receiving poor care. Confronting the employee directly about dealing with emotions may elicit a defensive response.
