When you place the call, state the reason for the call. Be brief and listen carefully. Repeat the test results and any orders the physician gives over the phone. Record accurately in the medical record the date and time the phone call was placed, the patient data reported, the name of the person spoken with, and whether an order was obtained. Charting telephone orders and documentation in the nurses' progress notes should be done as soon as possible after the phone call to prevent an entry by another caregiver before charting. The physician needs to countersign the order within a time frame as specified by the organization's policy.
